Description:
How the disaster started was that a foam piece flew out of the STS-107 and hit the left side of the orbiter. On some launches there were major or minor foam shredding instances, but they think Columbia was different and more serious. When the space shuttle re-entered the earth’s atmosphere it started to break apart. After the incident, space shuttle operations were discontinued for more than two years.
