apr 20, 2018 - Mr. Bowen refuses to sign travel voucher

I had to get my supervisor to sign a blank travel voucher that I needed signed by him for DOI as a part of their tax purposes. He was very rude. He said well why haven’t I gotten this request from anyone else. I said I it may be because they did not get their PCS packages until the next year. He said I needed to go back and ask the DOI people what they needed even after I showed him the email requesting the information. Supervisor refused to sign my travel voucher for tax purposes for DOI. Emails below.

In taxable year 2017, you had one or more Permanent Change of Station (PCS) vouchers processed which included a Withholding Tax Allowance (WTA).  Please follow the instructions on the attached Memorandum and submit the required documentation for processing by the deadline of July 13, 2018:
1.       Travel Voucher (OF1012)
2.       IRS – 1040EZ form (1 page only) or;
3.       IRS – 1040A form (2 pages)
4.       Statement of Income and Tax Filing Status for Tax Year form
 
As a reminder, please redact all Personally Identifiable Information (e.g. Social Security Numbers, Address, and Dates of Birth) from all forms and supporting documentation submitted to IBC. This information is not needed by IBC personnel.
 
**NOTE**
When completing the Travel Voucher (TV) the total amount claimed is to be blank.  The amount of the travel voucher reimbursement is not determined until the TV is processed by IBC, at that time IBC will input the correct amount.  You are required to sign and date in box 13 and your approving official (supervisor) will need to sign and date in box 14.  You may show this note to your supervisor as verification that they are signing a blank TV.

Hi Lee Ann. In 2017 I had a PCS voucher that was processed in 2017. It is my understanding that I need to submit some tax documents to DOI for their records. I was hoping to get some clarification regarding what documents needed to be submitted. It is my understanding that I needed the following documents: 
1. A Blank Travel voucher containing my name, my signature, and my supervisor's signature 
2. The first two pages of my 1040A 
3. My statement of income and tax filing for Tax Year 2017. 

Is this correct? My supervisor is uncomfortable with signing a blank travel voucher. Is there some other form of documentation that we could supply you with that would suffice instead of blank travel voucher? Thank you.
