Form A. Certificate for Insanity (dec 10, 1882 – dec 18, 1910)
Description:
a) 1877 Revised Statutes of Ontario, Chapter 220, Section 8, 9:
Name change from Form K to Form A. However, it is still required to have three certifying physicians.
b) 1882 An Act to amend the Act respecting Lunatic Asylums and the Custody of Insane Persons.
March 10th 1882
From 3 to 2 Medical practitioners required for certification.
«Section 3: Section eight of the above recited Act is hereby amended by substituting the word “two” for the word “three” in the third line thereof» R.S.O. c. 220, A. 8, amended.
Major modifications:
• It requires two medical practitioners for certification.
• The Form A aligns Certification in Ontario with the standards of New York State, England and Wales.
