Blue Form (jul 14, 1873 – mar 29, 1876)
Description:
The first record available for this form is July 14th 1873 (RG 10-268, B296098, Warrant n° 4078).After 1875 it becomes very rare. In 1876 there are only Schedule No. 2.
It was printed on blue paper and included 19 Queries that were intended to describe transferring prisoners’ insanity.
No major changes in comparison to the Yellow Form. Only two things to note is that the Patient is now referred to systematically as the Prisoner. Secondly, there is no dedicated space to be filled by the Gaol Surgeon at the bottom of the document.
