1. Behavioral
As a child Fish was in an abusive environment at his orphanage where pain was constantly present. This most likely led to Fish becoming a sadist and a masochist. Not to mention the Telegraph boy who got Fish into urolagnia coprophagia at a young age.
2. Biological
Fish was having many auditory hallucinations. He claimed to be talking to the apostle John from the bible. This points to Schizophrenia and or possible psychosis.
