It just stops.
Population declines dramatically, both because people are leaving their city-kingdoms and because there's not enough of them left behind to function properly, so they die.
Because there're no people, obviously trade stops, so there're no resources, which they'd come to rely on, so that tiny little percentage of people left also dies.
And yeah that's it. They're gone now.
