In 1842, Edwin Chadwick published a Report on the Sanitary Conditions of the Labouring Population, suggesting that poor living conditions caused poor health. Chadwick observed that in country areas the average age of death for workers was 38, but in towns like Liverpool it was only 15.

Chadwick's report led to the 1848 Public Health Act: a national Board of Health was set up, and local councils could set up their own boards of health, appoint medical officers of health, and collect taxes to make improvements to water supply and sewage. However, the Act was ultimately not very effective as it was optional, and most councils did not change.
