In 1869-1870, a form of 17 Queries is also attached to Warrants. These questions are mandatory for patients who are to be transferred to the Provincial asylum (Pictures).Here a transcription of a Form of the 1870s.
This Form is important because it introduced the idea that when prisoners are to be transferred to the Asylum, their insanity should be stated and declared according to several questions. In its original template it lasted only 4 years, from 1869 to 1873.
