What: Approximately 350,000 people were evacuated as a result of the accident, 8.4M people were expose to radiation and even today remediation is still on going.
Where: In Reactor No.4 at the Chernobyl Nuclear Power Plant. Surrounding areas Pripyat, Belarus, Chernobyl Raion, Kyiv Oblast, Ukrainian SSR, Soviet Union where also affected.
Why: The accident happened during a safety test, meant to measure the amount of steam needed to power the emergency feedwater pumps. Because of design flaws the users mistake it caused (a positive feed back loop) increasing the coolant temperature, this process resulted in steam explosions and melting of the reactors core.
