CLIENT: JOHNNY DEPP RN: ERIN BOERUM
6/26/16: RN in contact with client JD via text. Client states he is home in Los Angeles and would like to see RN. RN visited client at home. Client appeared well groomed. Client maintained good eye contact, appeared relaxed and comfortable. Client cooperative and focused. Affect appropriate. Speech rate normal, clear. Thought process and content normal and appropriate. Ct has several close friends visiting throughout the day, and states he has been having friends and family members stay with him to assist in emotional support. Ct displays good insight to current relationship difficulties and life situation. Ct is going through divorce with wife Amber H., and is dealing with the loss of his mother (passed away one month ago). Ct was provided with divorce request from wife AH three days after mother passed away. Ct states he was not aware AH wanted divorce, and expresses confusion regarding AH's desire to terminate marriage. Ct's divorce has been highly publicized and wife AH has been accused him of several character damaging allegations including domestic violence. Ct denies all accusations but is obviously saddened by current events. Ct is open with RN about emotions regarding divorce, media publicity, and mother's passing. Ct states he felt helpless previously, but is now angry at AH and wants to clear his name. Ct has hired legal team to handle divorce and allegations, and ct reports feeling less burdened by their assistance. RN assisted ct in processing feelings, and encouraged ct to continue utilizing individual therapy as he responded well to RN's use of therapeutic communication. Ct is resistant, stating, "I don't want anyone like Amber's (wife) therapist. He has only made things worse." RN gave emotional support, and educated ct on benefits of cognitive behavioral therapy and psychotherapy. RN offered to assist ct in Interviewing and finding an appropriate therapist, but ct declined at this time. Throughout RN's visit, ct maintains attention and Is actively participating in conversation. Ct consumed three vodka drinks during RN's 7-hour visit. Ct did not eat during visit, and drank water only with several prompts from RN. RN educated ct on possible interactions between prescribed medication and alcohol. Ct verbalized understanding but stated, "Right now, I need a little alcohol for all of the bad things I'm going through." RN provided 1:1 emotional support and encouraged ct to limit and/or abstain from alcohol and illicit drugs. RN encouraged ct to continue to notify her and Dr. Kipper of any changes in substance use or any behavioral/mental changes. Ct agreed to do so. RN and ct made plan to be in touch frequently for next several weeks until ct's current personal crises stabilized. Ct request to begin use of HGH Injections, which he has discussed with Dr. Kipper. RN notifled Dr. Kipper of ct's mental and physical status, and also of ct's request of HGH. Dr. Kipper will discuss HGH with ct.
