The Church of the Sacred Flame speculates:

Powerful arcanists ruling city-states that would become part of the Arcane Empire, made Blood-Pacts with ancient dragons and demonic entities to increase their power to new levels. Sorcerer-kings that made this blood pact (and their offspring) would have powers akin to demi-gods. ... but they would still be "cursed" with human nature. This newfound power would allow Sorcerer-kings to unite the various city-states into the Arcane Empire. The details of how this happened is lost to history.

The church believes this Blood-Pact necessitated the creation of Dragonborn and Tieflings. Why ancient dragons and demons required this cross-breeding is unknown, but since then Dragonborn and Tieflings spread across the Realm.
