Description:

In the 8 years prior to the incident, a small number of Anodyne personnel with knowledge and understanding of venteriobiology recommended on several occasions either a one-time inspection of every component within the primary inter-park infrastructure system - known as a 100 percent component inspection - or an upgrade of the material of construction of these key infrastructure components. The recommendations were not implemented effectively, and many key structural and mechanical components remained in service until failure on July 4th, 2007.
