Description:
The Apollo 25 Lunar Module suffered a partial decompression, injuring CDR Vance Brand and killing LMP Stuart Roosa. The mission never landed on Luna and instead aborted back to Earth. The mission was determined to be a "1 in 10 million" accident and Apollo 26 was quickly given a go-ahead.
