The form is dated January 10, 2013, but was not administered to Helfter until January 10, 2014 when he signed. He worked as supervisor on the Graveyard Shift for nearly a year before he was given his specific duties written out. Nonetheless he was still placed on the PIP with there never being specific instruction as to what his duties were and what was to be expected of him as supervisor.
