when Soviet martial of Artillery Mitrofan Nedelin ignored multiple safety warnings and ordered that a prototype R-16 Inter-Continental Ballistic Missile to be launched as a test disaster struck. the resulting explosion would lead to the death of 92 people, both military and civilian.
